MLTC (Managed Long Term Care) is a Medicaid managed care program for people who need long-term services and supports for an extended period—often in-home services like home health aides, personal care, physical therapy, or nursing.
People typically get assessed and enroll through NY Medicaid Choice or directly with a plan. Plans coordinate all your care including doctors, medications, and home services.
Adults and seniors who need help with daily activities (bathing, dressing, eating, mobility) for an extended time, and who are on Medicaid. You must need more than 120 days of services.
